Custom Forms

You can create custom forms or fields using the Forms module.

  1. Custom forms are independent forms that will appear in a different tab. Example: Smoking history, family history, pathology annotations, images, genomic tests, and so on.

  2. Custom fields: are fields that are embedded within the main screen

    1. Best suited for 2–5 fields to be added to the main screen.

    2. Supported only for participants, specimens, and visits.

    3. There can be only one custom field form per CP.

    4. CP-level forms will override the system-level forms.

Form Access

Any Super Administrator, Institute Administrator, and users with “Manage Forms” can access the Forms tab. They can create, edit, delete and link forms to other objects.
