Form Actions

Introduction

This page contains different actions that can be performed on forms like export, import, delete, etc.

Export

You can export the form XML using the below steps

Before v11.2:

  1. Navigate to Forms Module

  2. Search form by its name eg: PDX Transplant Conditions

  3. Click on the download icon to download the form XML.

After v11.2:

  1. Navigate to Forms Module

  2. Search form by its name eg: Specimen Details

  3. Click on the form.

  4. Click on Download XML option as shown below screenshot:

  2. This will download either an XML file or a zip file containing the XML file and PVs

Import

Once the form is exported, you can import the same form as it is on any instance of OpenSpecimen.

  1. Click the 'Import' button on the forms list page

  2. Choose the file and click on ‘Import’

  1. If the form is being edited, import the updated XML/ZIP. This will overwrite the form with the new changes from the imported file.

  2. If you want to create a clone of the previous form, you can export the form → Change the form name in XML → Import the updated XML.

  3. If you want to upload a file which had pvs folder and XML file, zip the contents of the folder(pvs folder and XML file) and import it.

Delete

You have the option to remove one or more forms if you no longer wish to collect data using those forms.

Delete a single form

To delete a single form, click on the delete button present under the form overview page

  1. Click on 'Delete'

  2. If there any data for this form, a pop-up with the number of records will be displayed like below.

  3. Click on 'Yes' to confirm form deletion. This will delete the form and all its records

Delete multiple forms

Multiple forms can be deleted at once.

  1. Select the forms and click on the 'Delete' button.

  2. Enter ‘DELETE ANYWAY' and click on 'Yes’

The delete button will only appear when you select one or more forms.

Audit Tracking

All updates related to form modifications are captured in the 'Audit Trail.' This includes changes such as editing or moving fields, as well as updates to form associations like adding or deleting attachments.

Form Audit

  1. Click on the ‘Audit Trail’ button from the form overview page

  2. A pop-up screen will display all the updates made to the form. The table has details like ‘Updated By' and 'Updated Time’ along with an option to preview or download the previous version.

  3. To view an older version, click on the eye icon beside the version.

  4. This will open a pop-up with the form in preview mode

  5. You can also download the older version of the form using the </> button

Form Association Audit

To check the past associations,

  1. Click on the 'Associations' button

  3. A list of all the previous associations will be displayed.
