Service Request Form

Some labs receive service request such as DNA or PBMC extraction for a specimen. To share these services information, user may want to print the service details into a PDF file. In such scenario, one can use specimen manifest feature to achieve this.

Example

A lab receives service requests such as DNA or PBMC extraction from other labs or site for a specimen, and they enter this information into the visit custom subform field named Request Service. For printing the Service Request information, they require it to be displayed only if:

1. Service Requested is DNA Extraction and either:

   • The Primary Tube label starts with P1, P4 or P5 or

   • The Specimen Type is Buccal Swab

2. Service Requested is PBMC Extraction and the Primary Tube label starts with P6

3. One page per service.
