This feature allows biobanks to create a 'Contact ‘Contact Us' form , which on the login screen. This can be used for any purpose like contacting the public to contact the biobank for a new project, new user training requestrequests, general queries, etc.

The form's contents can be configured per the biobank’s needs.

Configuration

1. Create a request form as per the requirement from using the forms cardForms module. Refer to Forms (Post v8.0) for more details about formsForms.

2. Create a new Catalog with a 'Request form' feature only and assign the new form as a request form.
   Navigate to the ‘Catalog’ → Fill in the mandatory fields like Title, Description, Item Details Query, Request Form, and Request Managers, and set the Display as 'Request Form’ only.
   Refer to Managing Catalogs for more details. and select “Request Form” in the “Display” field.

2. In the system ‘Settings'Settings', search for 'Contact us' and set the catalog id. You will get the catalog id in the menu bar of the catalog browser’s URL when you are on the new catalog's overview page.

3. When a user submits the Contact Us form is submitted, the response , an email notification is sent to the 'Request managers' in the catalog. The request looks like the one belowYou can also view past submissions within the catalog.
