Introduction

Biospecimens without quality annotations are of not much use. Every biobank might want to collect a different set of specimen annotations based on the disease of interest, clinical study, geography, etc. For example: smoking history, family history, pathology annotations, images, genomic tests, etc. Further, the biobanks might need additional fields on the standard data entry pages. E.g. Vendor name on the "create container page".

Using the "Forms" feature, you can create such forms without any IT support. The CP related forms can be attached to one or more protocols. The forms can be attached to the following objects:

2. Collection Protocols
3. Participant
4. Visit
5. Specimen
6. Specimen event
7. Containers
8. Distribution protocol
9. Projects
