Form Association Details

What is Form Association?

Users can attach custom forms at different levels in OpenSpecimen, such as participant, visit, or specimen, to capture additional data as needed. For example, a form like smoking history can be linked at the participant level.

Select 'Forms' from the navigation menu.
Navigate to a form of interest.
On the form overview page, click on 'Associations'.
Click on 'Add'.
Select level from the dropdown (e.g. Collection Protocol Custom Fields).
Click on 'Add'.
