Forms & Custom Fields

You can create custom forms or fields using the Forms module.

  1. Custom forms are independent forms that will appear in a different tab. Example: Smoking history, family history, pathology annotations, images, genomic tests, and so on.

  2. Custom fields: are fields that are embedded within the main screen (supported only for participant, specimen, and visits)

The forms can be attached to the following objects:

Form Access

Any Super Administrator, Institute Administrator and users with “Manage Forms” have access to the Forms tab. They can create, edit, delete and link forms to other objects.
