You can create custom forms or fields using the Forms module.
Custom forms are independent forms that will appear in a different tab. Example: Smoking history, family history, pathology annotations, images, genomic tests, and so on.
Custom fields: are fields that are embedded within the main screen
Best suited for 2-5 fields to be added to the main screen.
Supported only for participants, specimens, and visits.
There can be only one custom fields form.
If a custom fields form is assigned at the CP level, it will override the system level CP form.
The forms can be attached to the following objects:

Any Super Administrator, Institute Administrator, and users with “Manage Forms” have access to the Forms tab. They can create, edit, delete and link forms to other objects.
