Forms created using 'Custom formsForms' can be exported and imported to make copies on the same OpenSpecimen or across instances like test and production OpenSpecimen.

- Click on the form name in the forms list
- Open the form in edit mode and click on the 'Summary' tab on the right.
- Click on the export button

- Go to forms list
- Click on Create
- Click on Import and select the XML or Zip file.
- Save the form
- Go back to the forms list and attach the form to the appropriate level and one or more collection protocols.
