Description: This BO Template is used to add Custom Form data for Visits. The forms need to be created and attached at Visit level. Once a form is attached, user can access the template file by clicking on Collection Protocol→Import→selecting Protocol→ Select the CP → Import→Select 'Visit' under record type→ download template file
