  1. Custom forms are independent forms that will appear in a different tab. Example: Smoking history, family history, pathology annotations, images, genomic tests, and so on.

  2. Custom fields: are fields that are embedded within the main screen (supported only for participant, specimen, and visits)

    1. Best suited for 2-5 fields to be added to the main screen.

    2. Supported only for participants, specimens, and visits.

    3. There can be only one custom fields form.

    4. If a custom fields form is assigned at the CP level, it will override the system level CP form.
