  1. Go to participants list page of the collection protocol whose specimens custom form values has to be added/updated
  2. Click on 'More' button and select option 'Biospecimen Data'
  3. Select desired form, E.g. 'New Diagnosis Health Annotation' from 'Select Record Type' specimen's dropdown and click on 'Download Template File' to download the template file
  4. Enter all valid data in the template file as mentioned in the below 'Data Dictionary' table
  5. Upload the filled template file in 'Input Records File' field, select 'Import Type' as 'Create' to add or 'Update' to update values of the custom formsforms or 'Update Else Create' to either create or update record. For the 'Update Else create' feature please refer to Create & Update in Single Bulk Operation.
  6. Click on 'Validate and Import' button
  7. Refer 'Import status' to check the status of import job
