Introduction
Custom Forms allows to add site-specific data forms in OpenSpecimen. Super Administrators can build custom forms on their own to collect the additional data. This page will act as a library of commonly used forms like pathology and clinical annotations.
Forms Library
In NCI caTissue, standard clinical and pathology forms were created based on CAP(College of American Pathologists) checklist. These clinical and pathology forms were recreated using OpenSpecimen custom forms builder. The form files can be imported into OpenSpecimen instance and used as is or updated as per sites' needs.
The forms are uploaded on OpenSpecimen public demo site - http://demo.openspecimen.org.
